Our organization recognizes case management as a critical component of our system of community-based care for persons diagnosed with serious mental health issues. It is a seamless approach that ensures coordination and continuity of care, access to information, and the continued nurturing of our network of community resources for hi-risk and other identified individuals.

Service Components:

The major components of our Blended Case Management Services have been developed in order to assist recipients to achieve their stated goals and objectives, and to succeed within their community plan of care. These service components include:

• Intake and Screening
• Assessment and Reassessment
• Case Management Planning and Coordination
• Implementation and Ongoing Evaluation of the Case Management Plan
• Crisis Assistance and Intervention
• Monitoring and Follow-up Services
• Family Support and Empowerment
• Community Resource Development
• Provision of Services Dollars to Assist in the Maintenance of Basic Needs.

Referral Process:

All referrals for case management services must be processed through the Nassau County Department of Mental Health's Single Point of Entry/Accountability System. For more information contact:
